AUTHORIZATION FOR MEETING AND/OR COLLECTIVE BARGAINING

I, the undersigned, employed by the above Employer hereby authorize Painters' District Council No. 14, of the International Brotherhood of Painters and Allied Trades, a Labor Organization affiliated with the American Federation of Labor and Congress of Industrial Organizations, to act for me as my COLLECTIVE BARGAINING AGENT in meeting and/or negotiating with my present Employer or future Employers on all matters regarding my Wages, Rates of Pay, Hours of Work, Fringe Benefits, and all other conditions of Employment.

This full power and authority to act fort he undersigned SUPERSEDES AND CANCELS ANY POWER AND AUTHORITY HERETOFORE GIVEN to any person or Organization to represent me and shall remain in full force and effect for at least one year from the above stated date and shall continue year to year thereafter unless this authorization is withdrawn by written notice to the Union.
